Configuring a Signature Question

A Signature question allows users to provide an electronic signature as part of their submission.

This question type is commonly used for agreements, declarations, consent forms, certifications, and acknowledgements where a signature is required before submission.

Adding a Signature Question

  1. Navigate to the desired section of your form.

  2. Select Add Question.

  3. Select Signature from the Question Type dropdown.

  4. Configure the available settings.

  5. Select Save.


Configuration Options

Question Text

The question or prompt displayed to users.

Example:

  • Please provide your electronic signature.

  • By signing below, you certify that the information provided is accurate.

  • Please sign to acknowledge and accept the program terms and conditions.

Best Practice: Clearly explain what the user is agreeing to or acknowledging by providing their signature.


Mandatory Field

Determines whether users must provide a signature before submitting.

Enabled

  • Users must provide a signature.

Disabled

  • Users may submit without providing a signature.

Important: Signature questions are typically configured as mandatory when used for agreements, certifications, or acknowledgements.


Required for Draft Submission

Determines whether users must provide a signature before saving a submission as a draft.

Enabled

  • Users cannot save a draft without signing.

Disabled

  • Users can save a draft and return later.

Best Practice: Most programs leave this setting disabled, as signatures are often completed during the final submission process.


Help Hover Text

Additional instructions displayed when users hover over the help icon.

Example Uses:

  • Please review all information before signing.

  • Your signature confirms the accuracy of the information provided.

  • By signing, you agree to the program's terms and conditions.

Best Practice: Use Help Hover Text to explain the purpose and implications of the signature.


Hide Question From Reviewers

Determines whether reviewers can view the signature.

Enabled

  • The signature is hidden from reviewers.

Disabled

  • Reviewers can view the signature.

Common Uses:

  • Internal consent forms

  • Privacy-sensitive acknowledgements

  • Administrative agreements not required during review


Hide Question From Submitters

Determines whether submitters can view the signature question.

Enabled

  • The question is hidden from submitters.

  • The question can still be viewed and completed by administrators, if applicable.

Disabled

  • The question is visible to submitters.

Important: When using an E-Reference question type, this setting must be enabled. E-Reference questions are intended to be completed by reference providers and should not be visible to applicants.

  • Administrative acknowledgements

Best Practices

  • Place Signature questions near the end of the form after all required information has been completed.

  • Clearly explain what users are agreeing to before requesting a signature.

  • Use a Label question immediately before the Signature question to display any required terms, declarations, or instructions.

  • Configure the question as Mandatory when a signed acknowledgement is required.

  • Avoid requiring signatures for draft submissions unless absolutely necessary.

  • Consider whether reviewers need access to the signature before making it visible during the review process.

Important Notes

  • A Signature question is intended to capture an electronic acknowledgement from the user.

  • Signature questions are most effective when paired with a declaration, certification statement, or agreement.

  • If users must acknowledge specific terms, include those terms directly above the Signature question using a Label question.
